Date(s): January 3, 2019
Location of position: Wareham, MA

Community Connections Inc (CCI) is requesting proposals for a Grant Writer (individual or organization) to provide grant writing and proposal/application services to CCI.

Company Background:

Community Connections, Inc. (CCI) is a nonprofit organization that has been serving people with disabilities since 1985 with a mission "to provide life-enriching services that promote optimal independence for people with disabilities. Our extraordinary team supports each person in reaching their potential through goal-driven services and community partnerships." Over the last 33 years, we have grown to meet the needs of over 750 clients and their families in 65 towns and eight program facilities throughout Cape Cod and Southeastern Massachusetts.

CCI provides an array of comprehensive programming for people with disabilities ranging from 16 - 92 years of age. Our extensive services includes: day habilitation services, life skills and social skills training, school transition services, elder services, employment services/coaching, in-home support, and specialized transportation. For the youth entering our programs, we provide "Turning 22" Transition Services and Pre-Employment Transition services to high school students.

CCI program participants present with an array of complex medical, clinical, and physical conditions that require lifelong support. We offer programming that  includes a combination of clinical services and life skills/daily living training. For people with disabilities who are living with family members, our programs are often the only component in the community care system that provides these vital services. Without these services, families have no place to turn, and people living with a disability are left vulnerable to social, emotional and physical isolation. Using a personalized strength-based approach, we work with each client and their family to set measurable goals and objectives focused on growth, increased independence, and integration into the community.
