Transition- Turning 22

Individuals with intellectual and developmental disabilities face an exciting and challenging time in their lives as they are turning 22 years of age. This transition period marks the beginning of another chapter in life as they move from school programs.

Common questions for transitioning individuals include:

What is the minimum age to receive services?
When should I start planning for services for my loved one?
Where can I find services for my loved one?
Who can help me locate services?

Transition Services, from school to adult life, facilitate the change from one phase of life to another. There are many options for programs such as employment, day habilitation and life skills and our skilled Case Managers assess each individual’s needs for making a successful transition. They then assist students in implementing the process so that they have the skills and support necessary to live their adult lives as independently as possible.

Allow us to guide you and your loved one through this exciting time. Please fill out the form below to receive a free copy of our Turning 22 Guide.
