Supports
for a more active
independent life

Nursing Home Supports

For residents of long term care facilities with developmental or cognitive disabilities, specialized nursing home supports may be provided by Community Connections staff.  Services and activities incorporate the therapies recommended by our therapists as appropriate for each individual. Services may be provided within the nursing facility, at the Day Habilitation Program or in the community.  The services complement, reinforce, and are consistent with services provided by the Nursing Home as identified by their individual Pre-Admission Screening & Assessment Review (PASSAR) and individual support/service plan. Referrals for this program are generally made through Department of Developmental Services Area Offices or the UMASS Nursing Home Initiative.

For residents of long term care facilities looking to return to a community living situation, Community Connections staff can assist in providing transition services. These services include visits to potential new residences and community offerings.

What families are saying

“I would like to take this opportunity to give my thanks and gratitude to some wonderful people who care for my sister Lisa. Lisa is currently at a nursing home and Amanda cares for her on Sundays. She listens to music or reads with her and also tends to her feeding needs. This is comforting to Lisa and is greatly appreciated.
Many thanks to Laura who spends a great deal of time with Lisa. Laura does many things with her and keeps her stimulated and calm when the need arises. With Laura’s help we are able to take Lisa out in the community and do many other activities which would not happen if not for Laura’s help. Both of these women are a special gift sent by Community Connections and without them Lisa’s lifestyle would be very different. Many thanks and heartfelt gratitude.”

“Thank you is not enough for all you have done for my sister Judy. Maria, Kim and Carol, at a difficult and dark time in Judy’s life you were there to help in the nursing home to her move to a new facility and are still there supporting her in a new chapter in her journey. It has been difficult traveling from Middleboro to Fall River and knowing you are there eases our mind. Thank you for taking her to church and to other activities and keeping her active in the community”
