Nursing Home Supports
For residents of long term care facilities with developmental or cognitive disabilities, specialized nursing home supports may be provided by Community Connections staff. Services and activities incorporate the therapies recommended by our therapists as appropriate for each individual. Services may be provided within the nursing facility, at the Day Habilitation Program or in the community. The services complement, reinforce, and are consistent with services provided by the Nursing Home as identified by their individual Pre-Admission Screening & Assessment Review (PASSAR) and individual support/service plan. Referrals for this program are generally made through Department of Developmental Services Area Offices or the UMASS Nursing Home Initiative.
For residents of long term care facilities looking to return to a community living situation, Community Connections staff can assist in providing transition services. These services include visits to potential new residences and community offerings.
Nursing Home Supports provides specialized services to individuals residing in a nursing home.
- Self-Help skills including hygiene, eating and grooming
- Sensory-Motor skills including hand-eye coordination, perception and visual motor
- Occupational Therapy
- Physical Therapy
- Speech and Language Therapy
- Social skills including recreation and leisure activities
- Affective/behavioral skills including expressing emotion and cooperating with others
