Our Consumer Promise:
To provide personalized services that assist each individual to live successfully in their community.

The following practices and principles guide the services provided by Community Connections Supported Living Services:

  • Services or support that an individual receives shall change as his or her needs change.
  • An individual's preference shall guide decisions concerning where and with whom he or she lives.
  • Individuals shall have control over the environment within their own home.
  • The purpose of services and support to an individual is to assist that person to exercise choice in his or her life while building relationships with other individuals.
  • The services and support will be flexible and tailored to an individual's needs and preferences.
  • Services and support are most effective when provided where a person lives and within the context of day-to-day activities.
  • Relocation shall occur only when an individual prefers to move or when moving will improve services.

The Supported Living Services staff offer services within an individual's home that are tailored to his or her needs, while emphasizing increased independence and participation in the community. The Supported Living staff provide a high level of on-going personal care and safety assistance so that individuals can live in their own homes and direct their own lives with less than 24-hour support.

Staff encourage independence in tasks for which the individual has the skill and/or ability. Individuals are supported in living arrangements which are typical of those in which persons without disabilities reside.

Services Provided Include:

  • Assessing individual needs
  • Helping to secure an apartment or home of choice
  • Assisting with medical appointments and care
  • Assisting with grocery shopping, nutrition and meal preparation
  • Assisting with banking, bill paying, budgeting and/or becoming a representative payee   
  • Facilitating community participation through on-going coordination of social, recreational and educational activities in the community
  • Advocacy and Self-Advocacy facilitation
  • 24 Hour emergency response system
  • PCA Surrogacy

Accreditation
Community Connections is accredited at the highest level by The Commission on Accreditation of Rehabilitation Facilities (CARF) and is licensed and certified with distinction by the Massachusetts Department of Developmental Services (formerly DMR.)
