Community Connections offers services at convenient locations throughout Southeastern Massachusetts. Fulfilling its commitment to coordinate and arrange services that will allow you to live as independently as possible, the staff

Among the services available for your individualized plans are:
Day Centers - offering habilitation services with extensive therapies including speech, physical and occupational, nursing oversight and support, nutritional planning and medication management. At the Day Centers, clients participate in community and center-based activities such as volunteer programs, social and recreational events, musical participation and creative arts, day trips and special seniors programs.
In the Nursing Home Support Program, residents in long-term care facilities who qualify under the Rolland Class Action Suit receive specialized services from Community Connections staff, in coordination with the Nursing Home staff.
Employment - finding a job that is fulfilling as well as meeting your employer’s needs through vocational and skills assessment, job search and placement, job coaching and support.
Transition - facilitating the change from one phase of life to another such as attending school to pursuing a career, living with parents to living independently, relying on others for transportation to managing transit schedules or driving on your own.
Supported Living - supporting independent living by finding a place to live, providing services in your home including budgeting and household management, healthcare planning and nutritional guidance, coordinating community services essential to maintaining independence, with a 24-hour emergency response system.
Life Skills - for individuals who, with support, have the potential for greater independence at home and in their community; develop basic skills essential to living in your community through individualized, meaningful and supportive programs to increase your competence, confidence and independence.
Transportation - making mobility easier with a fleet of wheelchair accessible vehicles, providing transportation to and from the Day Centers, community activities and medical appointments.
All services are available at the Community Connections Center, located at 127 Whites Path in South Yarmouth, MA. In addition, one or more services are offered at other sites conveniently located throughout Southeastern Massachusetts. See the locations listing on this site.
