Day Centers

Our Consumer Promise:
To provide an extensive array of services combining traditional and alternative therapies, along with community activities and life skills training, to promote a healthy and productive lifestyle.

The Day Centers provide habilitation services that are designed to meet individual needs, from intensive therapies to community services. Day Centers help people achieve their goals by providing high quality care that builds on the strengths of the whole person.

Therapeutic Services
Therapeutic services include physical therapy, speech therapy, occupational therapy and behavioral therapy. Other therapies offered are music therapy and aromatherapy. Nursing oversight and support and physician consultations are also available.

Staff Ratios
The Day Centers have an average of one staff person for every five individuals served. One-to-one aides are available when a person’s needs are intensive.

Skills Training
Skills training is provided at each center and in the community. Included are daily living skills, health and safety skills, recreational activities and personal care.

Elder Services
Affiliations with local senior centers and Councils on Aging provide opportunities for day trips, lunches, dances, aerobics and volunteering. An aging specialist is on staff to help address individual needs.

Nursing Home Support
For residents of long term care facilites who qualify under the Rolland Class Action Suit, specialized services are provided by Community Connections staff.  Services and activites incorporate the therapies recommended by Community Connections therapists as appropriate for each individual.  They may be provided within the nursing facility, at a Day Center or in the community.  The services compliment, reinforce, and are consistent with services provided by the Nursing Home as identified by their individual PASSAR Assessment and individual support/service plan.
Referrals for this program are generally made through DMR Area Offices or the UMASS Nursing Home Initiative.

Auxiliary Services
Innovative communication devices are developed and utilized. Wheelchairs are repaired and maintained. Families and health care providers are trained on the use of adaptive equipment.

Transportation
A fleet of wheelchair accessible vehicles provides transportation to and from the Day Centers, community activities, medical appointments, social and recreational events.

Accreditation
Community Connections is accredited at the highest level by The Commission on Accreditation of Rehabilitation Facilities (CARF) and is licensed and certified with distinction by the Massachusetts Department of Developmental Services (formerly DMR.)

Hours of Operation
Day Centers are open Monday through Friday
from 9:00 a.m. to 3:00 p.m.
