To provide appropriate guidance and support to facilitate the change from school to adulthood.
Transition Services, from school to adult life, facilitate the change from one phase of life to another. Case Managers assess each individual’s requirements for making a successful transition. They then assist students in implementing the process so that they have the skills and support necessary to live their adult lives as independently as possible.
Typical transitions might be:
- Attending school to pursuing a career
- Living with parents to living independently
- Holding a part time job to working full time
- Relying on others for transportation to managing transit schedules or driving a vehicle
- Parental financial control to independent financial responsibility
Transition planning includes:
- Support services and skill building
- Employment and related training opportunities
- Connecting youth, teachers and mentors
- Academic and non-academic courses and learing experiences
- Help and experience in reaching personal goals
- Choices about where to live in your community
- Daily living skills and community socialization
Accreditation
Community Connections is accredited at the highest level by The Commission on Accreditation of Rehabilitation Facilities (CARF) and is licensed and certified with distinction by the Massachusetts Department of Developmental Services (formerly DMR.)
