Our Consumer Promise:
To provide an extensive array of services combining traditional and alternative therapies, along with community activities and life skills training, to promote a healthy, independent and productive lifestyle.
Life Skills Services provide individualized, meaningful and supportive programs that allow each member to increase their independence. The program is geared for individuals who, with support, have the potential for greater independence at home and in their community.
Therapeutic Services
Therapeutic services include physical therapy, speech therapy, occupational therapy and behavioral therapy. Other therapies offered are music therapy, art therapy, equine therapy, pet therapy and aromatherapy. Nursing oversight and support and physician consultations are also available.
Staff Ratios
The centers have an average of one staff person for every six individuals served.
Skills Training
Skills training is provided at each center and in the community. Included are daily living skills, health and fitness, safety skills, nutrition, meal planning, budgeting and shopping, recreational activities and personal care. Participants improve their knowledge and social skills, thus becoming more competent, confident and independent in their community.
Day Activities
Daytime activities help people access their community through opportunities for volunteering, day trips, art and aerobic/fitness classes and other community events.
Auxiliary Services
For individuals participating in Community Connections’ Employment Services, Life Skills Services provide a supportive, safe environment with personalized activities before or after work.
Transportation
A fleet of wheelchair accessible vehicles provides transportation to and from the Day Centers, community activities, medical appointments, social and recreational events.
Accreditation
Community Connections is accredited at the highest level by The Commission on Accreditation of Rehabilitation Facilities (CARF) and is licensed and certified with distinction by the Massachusetts Department of Developmental Services (formerly DMR.)
Hours of Operation
Life Skills Services operates Monday through Friday from 9:00 a.m. to 3:00 p.m.
